Xeomin vs. Botox: Which Is Better?

Botox is the household name in neuromodulators. Xeomin is the quiet, "naked" alternative that has earned a loyal following — including some of our regular patients at Prosper Health & Aesthetics in Sioux City. So which is better?

The honest answer: it depends on your goals, your history with neuromodulators, and which one your body responds to best.

Here's what to know.

What Both Botox and Xeomin Do

Both are botulinum toxin type A products. They temporarily relax specific facial muscles, which softens existing lines and prevents new ones from forming. Both treat:

  • Forehead lines
  • Glabellar lines (the "11s")
  • Crow's feet
  • Bunny lines on the nose
  • Brow lift, lip flip, masseter slimming, and other off-label aesthetic uses
  • Hyperhidrosis (excessive sweating)
  • TMJ and certain headaches when used therapeutically

Their mechanism of action is essentially identical.

What's Actually Different

Formulation. This is the big one.

Botox is botulinum toxin A bonded to accessory proteins (called "complexing proteins"). The proteins help stabilize the molecule but don't contribute to the clinical effect.

Xeomin is what's known as "naked" botulinum toxin A — the active molecule with the accessory proteins removed. This is the main difference.

Why does the protein matter? In a small subset of patients, the body can develop antibodies against those accessory proteins over time, which can blunt the effect of repeated treatments. Xeomin removes that variable.

For most patients, the difference is theoretical, not noticeable. For long-term users who feel like Botox isn't working as well as it used to, switching to Xeomin can sometimes restore the response.

Onset and duration. Very similar. Both start working in 3 to 5 days, peak at about 14 days, and last roughly 3 to 4 months for most patients.

Diffusion. Similar — both spread the same modest distance from the injection point. Technique matters more than product.

Cost. Both are competitively priced. At Prosper, our pricing is transparent and unit-based.

Who Should Choose Xeomin?

  • You've been on Botox for years and feel like it's not lasting as long as it used to.
  • You want a "purer" neuromodulator without accessory proteins.
  • You've had any allergic-type reaction to Botox in the past.

Who Should Stick With Botox?

  • You've been on Botox for years and it works beautifully.
  • You want the product with the longest track record and most clinical data.
  • Your injector knows your face inside and out on Botox dosing.

What About Daxxify?

Daxxify is a newer neuromodulator that often lasts 6 to 9 months for many patients — significantly longer than either Botox or Xeomin. We offer Daxxify at $6 per unit and it's increasingly popular for patients who want fewer touch-ups per year. The unit conversion is different, so a "Daxxify treatment" usually requires more units, but the cost-per-month often comes out lower.

The Most Important Variable

The single biggest factor in how good your neuromodulator results look isn't the brand — it's the injector. Placement, dose, and a real understanding of your face are what make the difference between "natural and refreshed" and "frozen or asymmetric."
